CHILDREN FAMILY SERVICES » Make a referral

Boost Collaborative accepts referrals from anyone with a concern about a child's development, as long as that referent has parent permission to do so.  The majority of referrals come from physicians.  Other common referents include early childhood professionals, hospitals, and other medical providers.

A referral is considered active when the referent has permission to share information with Boost Collaborative and is able to provide basic information about the child (date of birth, developmental concerns, medical status) and parent contact information.  Once this basic information is provided a Family Resources Coordinator

contacts the family, usually within 48 hours.

Parents are also encouraged to self-refer whenever they have a concern about their child's development.  This can be accomplished by simply calling Boost Collaborative at 509-332-4420.
